Healthcare Assistant Out-of-Hours

Job Introduction

As a Healthcare Assistant within our Out of Hours community nursing team you will be working 2 nights per week between 10pm - 8am, where you will work in pairs with a Community Nurse Team Leader, responding to calls overnight and visiting patients in their own homes.  

This role offers you the flexibility to work on days that suits you and the chance to work additional hours.

Our work is crucial to the community as we aim to prevent unnecessary hospital submissions and care for people in their hoes with a range of conditions including wide range of conditions including diabetes, wound and end of life care. 

Main Responsibility

As part of our community nursing team we empower you to work autonomously and with flexibility by providing you with an ipad to be able to check in from home, write notes on the go and allow you not be trapped in an office base. 

Responsibilities include but are not limited to:

  • To assist in providing nursing care as delegated by the Senior Sister/Team Sister within patient’s homes.
  • To maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
  • To deliver care in accordance with organisation policies, procedures and guidelines, according to competencies, as delegated by the Senior Sister/Team Sister
  • To report any changes in condition of the patient to the Senior Sister/Team Sister or in their absence the designated nurse in charge of the caseload.

The Ideal Candidate

Ideally you will have previous experience of working within a care setting and be a naturally empathetic individual, with the ability to build strong relationships with service users and their families, as our aim is to deliver positive experiences to patients at every stage of their healthcare journey. 

As this is a community role, it is essential that you have a valid UK driving licence and access to a vehicle for work purposes.
