Job Introduction
We are looking for someone to join our Integrated Discharge team as a Coordinator within Gravesham Community Hospital to provide business support to clinical and non-clinical staff!
As part of the Integrated Discharge team, you will work alongside Nurses, other healthcare assistants and administrators who are a key part of supporting the assessment and discharge planning of patients with complex needs. They ensure that the team provides a high quality, effective specialist service to patients and their families and carers.
We have both a full time and part-time opportunity available for either 37.5 or 22.5 hours across per week. Working hours are typically Monday to Friday between 9-5pm, however occasional weekend work will be required between the hours of 10-4pm. There will also be a requirement to work bank holidays, where we also offer enhancements for working unsocial hours and weekends. By working unsocial hours and Saturdays you will receive your basic rate of pay plus 33% and by working Sundays and Bank holidays you will receive your basic rate of pay plus 50%!
Main Responsibility
- To act in accordance with the Integrated Discharge Team in processing new referrals. This includes but is not limited to screening and allocating referrals
- Processing referrals onto allocated pathways, in a timely manner, cross referencing with the caseloads and signposting appropriately across the organisation
- Engage, attend and provide updates in daily IDT meetings, in relation to patient discharges
- To communicate with all members of the multidisciplinary team when a concern is raised regarding a patient’s discharge, escalating to the team lead or clinical service manager as necessary
- Liaise with individual care agencies and KCC/ social services department regarding packages of care and placement for patients, ready for discharge as required
The Ideal Candidate
We are looking for someone with previous administrative experience in a busy, customer facing environment, preferably in a healthcare, education or social care setting.
The ideal candidate will have effective interpersonal, organisational and communication skills and can work with discretion, sensitivity and maintain confidentiality. You will also be able to prioritise your own workload and contribute to the changing demands of the service.
We would like someone who works well as part of a team and is willing to undertake training relevant to the role.
