We have an exciting opportunity to join our Community Nursing Service as a Band 3 Community Health Care Assistant currently based at Orbital in Swindon.
You will join an innovative and forward-thinking multidisciplinary team of Community Nurses and Healthcare Assistants where individual development, and progression is key.
As the way community services is delivered changes, our ethos is to provide a responsive patient centred approach. You will work seamlessly in collaboration with a variety of community services with the opportunity to provide care for a wide variety of patients.
We have a strong focus on flexible and agile working balanced alongside provision of high-quality services. We provide a 7-day service 07:00- 22:00 with an expectation that your working pattern will support this. This role offers the opportunity to flex across our community teams including Swindon End of Life Care Team (SELeCT) urgent community response, community nursing and Hospital at Home. Ensuring our patients receive the right care, by the right person, in the right place, at the right time is at the heart of what we do. Utilising this approach, we believe will enable you to maximise your clinical skills, development opportunities and job satisfaction.

As a Community Health Care Assistant for the Swindon Community Team, you will mainly cover the Swindon area. A full UK driving licence and access to a vehicle for work purposes is essential.

Main Responsibility
As a Community Health Care Assistant, you will;
- Deliver care/treatment within a community setting, delegated by a registered practitioner and following an appropriate care plan.
- Keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
- Be proficient in the use of electronic patient record system, have excellent communication and documentation skills that are clear, concise and accurate.
- To work independently within the community setting, visiting patients within their own home.

The Ideal Candidate
Essential
- Good general education.
- Care Certificate – or willingness to achieve.
- Experience of working under supervision.
- Experience of supporting other workers.
- Ability to work as a team member.
- Delivering patient focused care.
- Ability to recognise own limitations.
- Typing/data inputting skills.
- Physical skills to manually handle patients and use appropriate lifting aids.
- Undertake specific nursing/therapy skills.
- Driven to achieve.

- Experience of community working
- Experience of the NHS.
- Experience of work in an acute or urgent health care setting
- Basic wound care knowledge and skills
- Current full driving license and use of a car during working hours.
