Job Introduction

As a Care Co-ordinator you’ll join a team who are focused on delivering impeccable customer service to our staff and service users to really make our service users and staff feel the difference. 

HCRG Care Group provides community services across the West Lancashire area, with the Single Point of Access Team being an integral part of this service.  As a Care Co-ordinator, you will provide excellent customer service to patients, GPs, etc who access this community hub.

Main Responsibility

To provide an excellent customer experience for patients and professionals contacting the Single Point of Access (SPA) Team by telephone and email as the first point of contact to HCRG Care Group Community services in West Lancashire.  Ensuring effective filtering and directing telephone calls for services across Community services in West Lancashire and where required with partner organisations.

Supporting clinical teams by booking and scheduling routine clinic activity based on rules and protocols and signposting other activity to the relevant services within required timescales.

The CCH is operational 8am – 8pm, 7 days a week

 

The Ideal Candidate

You will have

  • Good all-round basic education to at least GCSE level or equivalent, including maths and English
  • Experience within a customer service role.
  • Ability to work as part of a team
  • Excellent interpersonal and communication skills, both verbal and written
  • Polite and helpful customer service skills
  • Good telephone manner

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
