Band 7 Occupational Therapist

Job Introduction

We are seeking an experienced and motivated Registered Nurse, Occupational Therapist or Physiotherapist to lead our Community Transfer of Care Hub (CTCH).

This is a pivotal Band 7 leadership role for a discharge specialist with expert knowledge in complex discharge planning, patient flow and system-wide working. You will lead and manage a team supporting patients with complex needs to transition safely from hospital into community placements, packages of care or community beds.

The role requires strong clinical credibility, confident decision-making and the ability to work collaboratively across organisational and professional boundaries.

Main Responsibility

As Team Lead, you will:

  • Lead and manage the CTCH team on a day-to-day basis

  • Act as a discharge lead specialist for complex cases across the system

  • Support safe and effective Pathway 1 and Pathway 3 discharges

  • Monitor delayed transfers of care and drive service improvements

  • Take responsibility for the most complex and high-risk cases, including safeguarding and homeless patients

  • Investigate failed discharges and embed learning across the service

  • Provide clinical oversight for community placements

  • Work collaboratively with acute trusts, GPs, social care, ICBs and partner agencies

  • Lead audits, governance initiatives and quality improvement activity

  • Support staff development, supervision and appraisals

  • Promote evidence-based practice and uphold clinical governance standards

You will play a key role in managing bed pressures, improving patient flow and ensuring patients and families are actively involved in discharge planning.

The Ideal Candidate

We are looking for a confident clinical leader who can manage complexity and influence across the wider system.

Essential:

  • RN Degree / Occupational Therapy / Physiotherapy qualification

  • Active NMC or HCPC registration

  • Significant post-registration clinical experience

  • Experience in complex discharge planning

  • Experience leading or managing staff

  • Ability to manage conflict and make decisions under pressure

  • Strong communication skills across multi-agency environments

  • Experience initiating and implementing service change

Desirable:

  • Management qualification

  • Teaching/assessing qualification

  • Leadership programme completion

  • Experience of financial management
