About the Role

We are looking for an experienced and motivated Occupational Therapist to join our Urgent Care & Falls Response Service.

The role involves providing rapid holistic assessment and intervention for patients presenting with urgent care needs, frailty, reduced mobility, functional decline and falls within the community setting. You will work closely with multidisciplinary colleagues to support admission avoidance, facilitate safe discharges and maximise patient independence.

As part of the service, you will also support the Single Point of Access (SPOA) function through triage, referral prioritisation and clinical decision making to ensure patients receive timely and appropriate care.

This is an autonomous and rewarding community role that requires excellent clinical reasoning, communication and organisational skills.

Main Responsibilities

  • Deliver Occupational Therapy assessment and intervention within patients’ homes and community settings
  • Support urgent community response and falls management pathways
  • Complete holistic functional assessments and develop patient-centred treatment plans
  • Work collaboratively with multidisciplinary teams, GPs, social care and partner agencies
  • Contribute to SPOA triage and referral management processes
  • Support hospital admission avoidance and timely discharge planning
  • Provide advice, education and support to patients and carers
  • Supervise and support junior staff, rehabilitation assistants and students where appropriate
  • Contribute to service development, audit and quality improvement initiatives

What We’re Looking For

We are looking for someone who has:

  • Degree qualification in Occupational Therapy
  • HCPC Registration
  • Experience working within community, rehabilitation, urgent care or intermediate care services
  • Strong understanding of frailty, rehabilitation, urgent care and falls management
  • Excellent communication and clinical reasoning skills
  • Ability to work independently and manage a varied caseload
  • Passion for delivering high quality patient-centred care
  • Full UK driving licence and access to a vehicle for community visits
