Band 4 Associate Practitioner

Job Introduction

We have an exciting opportunity for a Associate Practitioner to join our Dartford PCN Team, based at Livingstone Hospital, Dartford.

This is an ideal opportunity if you are an experienced rehabilitation/therapy assistant looking to develop your knowledge and skill set, as we offer professional development through an Associate Practitioner Apprenticeship,and you have a strong passion for leading group exercise sessions. 

Due to the travel included in this role, a Full UK driving license with access to a car is needed to carry out the duties of the post.

Main Responsibility

As a Band 4 Associate Practitioner you will be:

  • Ensure correct use of confidential information and sensitive information in line with Caldicott and Information Governance Policies
  • To participate in the ongoing development and evaluation of the pulmonary rehab pathway.
  • To undertake clinical observations including blood pressure, respiratory rate, temperature, heart rate and SATS.
  • To undertake blood tests as requested from registered clinicians/ GP’s/Consultants, following training in accordance with organisational policy.
  • Communicate effectively so that the appropriate member of the MDT is aware of any change to a patient’s needs or problems.
  • Provide appropriate professional with up to date accurate information regarding patient’s progress.
  • Be able to independently evaluate progress of treatment and provide consistent feedback (verbal/written) to qualified practitioner.
  • Be able to deal with sensitive and emotive situations/information which may be distressing to individual patients using appropriate verbal communication skills.

The Ideal Candidate

We are looking for someone who has previous experience working as a band 3 rehabilitation/ therapy assistant, in an NHS community rehabilitation setting and has the ability to operate community equipment and assess needs and implement care. You will possess the ability to undertake nursing skills and prioritise visits in accordance with patient needs and can work well as a team but can organise their own workload without direct supervision.

This role would suit someone who is currently an associate practitioner or someone who is a band 3 looking to take the next step in their career. We welcome applications from candidates who are currently studying due to qualify.
