Job Introduction

We are looking to appoint a “Ward Specialist Doctor (Community Hospital)” where you will be working as part of Bath & Northeast Somerset NHS Adult Community Services. This position will be based at Sulis Ward, St Martins Hospital, Bath. The role provides medical care for a wide variety of complex problems with an emphasis on rehabilitation of adult patients. The ward is comprised of 29 ‘step down’ beds, a number of which are used for patients on the stroke pathway. Most patients are transferred from the Royal United Hospital, Bath but some can also be admitted directly from elsewhere.

As a Ward Specialist Doctor, you will need to be an approachable source of knowledge for your colleagues as well as making conscientious decisions in relation to challenges presented to you in the medical care and holistic management of the patients on the ward. The role is for 16 hours per week, worked over 2 days per week and must include a Friday.

You will be joining a multidisciplinary team where you will be working alongside nursing and therapy colleagues and have the in-reach support of consultant stroke and geriatrician physicians from the Royal United Hospital. HRCG Services Limited has been commissioned by the Bath and Northeast Somerset, Swindon & Wiltshire Integrated Care Board to deliver the Bath and Northeast Somerset Community Health and Care Services.

As well as providing services locally, we also deliver other community services provided by the NHS and voluntary organisations to improve the physical and mental health and well-being of the Bath & North East Somerset population and work collaboratively with stakeholders and provider partners as part of the newly formed BaNES, Wiltshire, Swindon Integrated Care System.

Main Responsibility

Responsibilities include but are not limited to:

  • To provide medical input for the inpatient unit at St Martins Hospital Bath – some of which are stroke rehabilitation beds & the remainder providing step down capacity from local acute hospitals
  • To provide some cross cover at times to John Stacey Ward at Paulton Memorial Hospital
  • To complete a comprehensive assessment of all new patients admitted to the ward as soon as possible
  • Formulate a problem list and differential diagnoses, organise any necessary investigations and document management plans in the MDT patient record
  • Work closely with the MDT, Consultant Stroke Physician, Geriatrician, and speciality Doctor, participating in weekly MDT meetings and ward rounds
  • To refer on to other specialties or clinics as appropriate
  • To advise on management of patients with acute intercurrent problems arising during their rehabilitation
  • To liaise with out of hours service regarding any patients seen in the evenings or at weekends and Bank holidays
  • Complete discharge summaries including medication and on-going treatment and management plans following discharge
  • Participate in clinical audit and service improvement projects as directed by the service lead

The Ideal Candidate

You will be GMC registered with a license to practice and have a strong clinical background working within Care of the Elderly.

A minimum of 4 years postgraduate experience which will include experience of assessing and treating elderly patients in an inpatient setting.

Confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e., Outlook, Word, Excel.

Essential Criteria

  • Full GMC registration or eligibility to be included on the register and holding a current license to practice at the time of appointment
  • Minimum of 4 years post graduate experience (or equivalent of part time/flexible basis)
  • Currently revalidated & up to date annual appraisals
  • Experience of assessing and treating people in an inpatient setting and as part of an MDT
  • Experience of treating elderly patients and long-term conditions
  • Evidence of post graduate CPD and ability to implement new developments and treatments
  • Awareness of current relevant issues in service provision, policy, and legislation
  • Understanding of the importance of clinical governance within the organisation and for patient care
  • Evidence of audit and or research to support quality improvement
  • Evidence of excellent communication skills
  • Commitment to professional development
  • Knowledge of the NHS
  • Understands the importance of evidence-based practice and clinical effectiveness
  • Understanding of Clinical Governance and quality issues as well as health and social care policy
