Transitions Social Worker

Job Introduction

We are looking for a passionate Transitions Social Worker to join our integrated Health and Social Care organisation, to join the Specialist services including Learning Disabilities in Radstock. This role will have a particular focus on supporting young people up to the age of 25 who are transitioning from Children’s to Adult Services.

We are also offering a welcome bonus of £2000 to the successful applicant 

As a Transitions Social Worker you will be responsible for carrying out the delegated responsibilities given to us by BaNES Council, namely: to undertake first adult care assessments and reviews with young people up to 25 with a learning disabilities diagnosis, and their carers, and to work with service users to create care and support plans which meet their needs.

You will join a friendly and extremely supportive team of Social Workers, giving you the opportunity to work within a multi-disciplinary role together with Psychologists, Occupational Therapists, Speech and Language Therapists and education professionals.

We have multiple vacancies and so have both full-time and part time opportunities. Service operating 9am-5pm hours, there is room for flexibility within this upon discussion.

On 1 April 2024, Adult Social Care and Learning Disability services will transfer to Bath and North East Somerset Council with TUPE protection. We are collaborating to ensure a smooth and safe transition of services and staff in all eligible posts 

Main Responsibility

As a Social Worker you will manage a caseload which will include complex cases and will work independently and effectively, under the general supervision of a Team Manager.

You will carry out the delegated responsibilities given to us by BaNES Council including:

  • Undertaking assessments and reviews of people with care and support needs and carers in accordance with the principles of the Care Act 2014
  • Creating care and support plans which appropriately meet the needs of people with care and support needs and carers.
  • Undertaking Mental Capacity assessments and, where appropriate, complete Best Interests Decisions in accordance with the Mental Capacity Act 2005.
  • Undertaking risk assessments and work to reduce risk or engage with the person and MDT in well considered positive risk taking strategies.

The Ideal Candidate

You will:

  • Be a qualified Social Worker (Social work degree, Diploma in Social Work, CQSW or equivalent)
  • Registered with Social Work England
  • Have a Full UK Driver’s License and access to a car

Ideally you will have had experience working with wider range of adults including people with Autism, Mental Health Conditions or Learning Disabilities.
