Team Sister - Community Nursing

Are you an experienced Community Nurse looking for the next step in your leadership career?

We are looking for an enthusiastic and motivated Band 6 Team Sister/Charge Nurse to join our Community Nursing team in Sittingbourne.

This is an exciting opportunity to combine hands-on clinical care with leadership responsibilities, supporting patients with a wide range of complex health needs in their own homes. You'll play a key role in helping people manage long-term conditions, avoid unnecessary hospital admissions and receive high-quality care within the community.

As a senior member of the team, you'll manage a defined caseload while supporting the day-to-day coordination of care delivery. You'll provide clinical leadership, support colleagues with decision-making, contribute to triage and caseload management, and help ensure the service delivers safe, effective and patient-centred care.

You'll also work closely with GPs, social care teams, specialist services and other healthcare professionals to ensure patients receive seamless care that is tailored to their individual needs.

This is a full-time position (37.5 hours per week), and the service operates between 8am and 8pm, 7 days a week, including weekends and bank holidays.

A full UK driving licence and access to a car for work purposes are essential.

Main Responsibilities

As a Band 6 Team Sister/Charge Nurse, you will:

  • Manage a defined caseload of patients with complex nursing and health needs within the community
  • Assess, plan, implement and evaluate individualised care plans to support positive patient outcomes
  • Support patients with long-term conditions, palliative and end-of-life care, wound management and other community nursing interventions
  • Participate in the triage of referrals and support effective allocation of visits according to clinical need
  • Provide clinical leadership and act as a role model for high standards of patient care
  • Support the Senior Sister with the day-to-day operational running of the service
  • Lead, supervise and support junior colleagues and students to develop their knowledge, skills and confidence
  • Participate in staff development, supervision, appraisals and competency assessment
  • Work collaboratively with GPs, social care teams, specialist nurses and wider multidisciplinary colleagues
  • Promote patient safety through effective risk assessment, safeguarding practice and clinical decision-making
  • Contribute to quality improvement, service development, audit and clinical governance activities
  • Maintain accurate and timely patient records using electronic systems
  • Support the delivery of evidence-based practice and high-quality care across the team
  • Participate in the service's 7-day working arrangements, including weekends and bank holidays

The Ideal Candidate

Our ideal candidate will be an experienced and compassionate Registered Nurse who is passionate about delivering high-quality community care and supporting the development of others.

You will have:

  • Current NMC registration
  • A Teaching and Assessing qualification or equivalent
  • Evidence of ongoing professional development
  • A minimum of 3 years' experience as a registered nurse
  • Experience within both community and acute or hospital-based settings
  • Experience supporting patients with long-term conditions and complex healthcare needs
  • Experience supervising, supporting or leading colleagues
  • Excellent clinical assessment and care-planning skills
  • Strong communication and interpersonal skills
  • The ability to prioritise and manage a changing workload effectively
  • Good understanding of safeguarding, risk management and clinical governance
  • Strong IT skills, including electronic patient record systems
  • The ability to work autonomously and make sound clinical decisions
  • A commitment to delivering high-quality, person-centred care

Ideally, you'll also have experience in staff appraisals, clinical supervision, service improvement activities or leadership responsibilities within a community healthcare setting.

As this role involves travel throughout the community, a full UK driving licence and access to a car for work purposes are required.
