Team Leader- Community IV Team

Job Introduction

As a team leader you will provide effective management and clinical leadership to the Community IV team to ensure the safe delivery of care to patients requiring IV treatment either in the clinic or home environment.

Main Responsibility

Main duties of the job

  • To work in partnership with the local Commissioners, GPs and Social Care colleagues to ensure the teams deliver and support the effective flow of patients between primary community intermediate and secondary 
     care.
  • To ensure the safe delivery and administration of intravenous therapy treatments in the clinic or home environment as per policy and protocol.
  • To work with the Community Pharmacy Lead and medical teams on future clinical pathways that may develop to support people to remain in the community setting for delivery of their care. 
  • To ensure that clinics operate within the scope of the service specification to ensure clinics are used effectively to maximise the potential of patients that can be cared for in the clinic throughout the day. 

To work closely with acute services such as SDEC to support and facilitate patients to undergo their treatment in a clinic setting or at home

  • To effectively operate and manage a community team.
     Develop and maintain positive relationships with key stakeholders, including patients and the public all members of staff, local GPs and practice staff, colleagues from across the 3rd sector, commissioners and social care staff.
  • To provide excellent leadership for a community team, including all staff within the team.
  • To provide effective operational management of the team ensuring that resources available are utilized effectively and meet patient needs.

The Ideal Candidate

  • Registered Nurse  
  • Educated to post graduate level or to have equivalent knowledge and experience
  • Management Qualification or working towards one
  • Leadership programme
  • V300 independent and supplementary Prescriber.
  • Evidence of successfully leading a team
  • Experience of resource management
  • Experience of managing HR issues
  • Experience in delivering agreed change agendas
  • Experience in delivering clinical governance, risk and safety management
  • Experience in undertaking investigations into complaints and clinical incidents
  • Proven ability to work with colleagues from across the multidisciplinary and multi-agency team
  • Good understanding of finances and budgetary management 
  • Good  understanding of workforce and performance management
  • Understanding of national agenda and opportunity for community services
  • Excellent understanding of community services
  • Good working knowledge of GWH Trust business practices
  • Good communication, influencing and negotiation skills
  • Ability to motivate and lead a team
  • Ability to work in a pressurised environment , balancing competing agendas
