The Bath Social Care Team are looking for passionate individuals to join a team who are focused on excellent social care, where you are an integral part of our multi-professional provision in Adult Social Care.
The team is experienced and supportive and consists of Social Workers, Social Care Practitioners, Occupational Therapists and an Admin team.

Alongside this, as a social worker you will receive: Regular supervision, support, peer supervision and team touchdown meetings: Training includes access to legal masterclasses - subjects like trauma informed practice, strength and asset based practice, having 'difficult discussions and chairing meetings. ASYE social workers are supported with regular month training and learning sets, peer support across the organisation.

Main Responsibility
Responsibilities include:
- Assessing the needs of service users and carers, taking care to understand the individual’s priorities and circumstances,
- Planning, implementing and evaluating outcomes, working with and for individuals.
- Identifying and delivering genuine person-centred, strengths-based support,
- Connecting service users and their families with suitable community resources and services.
- Supporting people to make their own decisions, championing the principles of choice and control to have services organised and risks managed.
- Working closely with the community and partner agencies and have the ability to understand local needs by supporting individuals in a positive and responsive way.
- Maintaining your professional standards as our service continues to develop
- You will work in line with best practise legislation to promote the wellbeing and dignity of service users
- Take necessary and statutory action in accordance with Safeguarding procedures to protect vulnerable adults.

The Ideal Candidate
We will expect you to be highly motivated, passionate about social care and committed to your own continuous professional development. You will also need to be able to work under pressure, be legally literate and have a good understanding of the Care Act (20140 ) and the MCA(2005), strengths based and personalised social work.
We are looking for someone who is flexible and adaptable in their practice, someone who is creative, innovative and searches for imaginative solutions that are not always obvious. You need to be self-motivated, pragmatic, and resilient.
You must be qualified with a social work degree or equivalent and be a registered social worker with Social Work England: or will be newly qualified and looking to gain invaluable experience with our team.

You will be based at our St Martins Hospital, Bath base where we have both Full time (37.5 hours per week) and Part time (18.5 hours per week) options.
