Social Care Practitioner

Job Introduction

We are looking for a Social Care Practitioner to join our First Response team in Bath and North East Somerset.

As a Social Care Practitioner in our First Response team, you will have the opportunity to work alongside social workers and other professionals to support adults with physical or learning disabilities who may be experiencing social, emotional or practical challenges in managing their daily lives.

This is a full-time opportunity to join a fantastic team where your role will play a key part in maximising the health, wellbeing and independence of individuals in the Bath and North East Somerset area. You will undertake Care Act 2014 assessments on behalf of the Council for adults and carers and determine eligibility for funded support via a personal budget to meet individual needs.

The hours are typically 9-5 and you will be expected to work from the office with flexibility to work from home.

Main Responsibility

  • To gather information about an individual’s presenting circumstances, identify any immediate risks and plan actions and interventions accordingly
  • Undertake short term support assessments, Care Act  and Carers’ Assessments to determine eligibility for funded social care support
  • Monitor the quality of care provided and review support plans in the community and residential and nursing home placements
  • Undertake straightforward Mental Capacity assessments and, where appropriate, complete Best Interests Decisions in accordance with the Mental Capacity Act 2005

The Ideal Candidate

We are looking for someone who has experience working directly with older people or those with learning disabilities who can also demonstrate effective verbal and written communication. The ideal candidate will possess knowledge and understanding of the legislation that applies to social care practice and a good working knowledge of safeguarding adults’ policies and issues.

While this is not essential, we would welcome applications from those who have experience of assessment, support planning and undertaking reviews, working in an empowering and person-centred way. It is also desirable if you have experience of applying eligibility criteria/ thresholds and a good understanding of ‘strengths based’ principles.
