The Bath Social Care Team are looking for a passionate individual to join a team who are focused on excellent social care, where you are an integral part of our multi-professional provision in Adult Social Care.
As an Adult Social Care Community Team we work with a wide range of people with varying presenting needs. This provides excellent opportunities for developing social work skills and knowledge. The team is experienced, supportive and reflective and consists of social workers, social care practitioners, occupational therapists and administrators.
The ideal candidate will be someone who has experience of working in a health and/or social care setting and who has knowledge of the Care Act; and the ability to apply it in practice. The team are focussed on professional growth and development, and will ensure you feel valued every day.

Main Responsibility
Your days will be rewarding and you will be undertaking Care Act and Mental Capacity Act assessments, support plans and reviews with people who have physical disabilities or other complex needs.
This role will involve working with families, carers and other professionals, agencies and organisations across the World Heritage site of Bath.
The work is focused on promoting independence, enabling the individuals we work with to remain in their own homes and reduce the need for hospital admission or long term care.
The Ideal Candidate
You will understand what really matters to our service users, families and their carers working closely with the community and partner agencies and have the ability to understand local needs by supporting individuals in a positive and responsive way.
You will be committed to maintaining your professional standards as our service continues to develop and enable the creative use of resources including assistive technology.
You will have a relevant qualification equivalent to NVQ Level 3 or equivalent Health and Social Care experience. Training related to Health and Social Care assessments and experience of managing a caseload including working with people experiencing a range of physical disabilities, medical, mental health difficulties, substance misuse and alcohol needs and learning difficulties.
Evidence of good interpersonal skills, good verbal / written communication skills. You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
