Job Introduction

Join our integrated Health and Social Care team to play a pivotal role in meeting the diverse needs of Bath and North East Somerset residents!

As a vital part of our integrated Health and Social Care organization, the Senior Practitioner within HCRG Care Group plays a pivotal role in meeting the diverse needs of the people of Bath and North East Somerset. Our Senior Practitioners are entrusted with delegated responsibilities by B&NES Council, encompassing assessments, reviews, care planning, and safeguarding activities under the Care Act 2014 and Mental Capacity Act 2005.

In addition to these core duties, Senior Practitioners take on leadership responsibilities within the team, including supporting newly qualified colleagues and fulfilling various key roles as required.

Main Responsibility

  • Lead with Impact: Provide leadership and assist the Team Manager in managing team tasks, including workload prioritization, monitoring, and review.
  • Mentor and Guide: Supervise team members and provide practice consultation to support their professional growth.
  • Empower Lives: Conduct assessments and reviews in accordance with the Care Act 2014, creating care and support plans that empower individuals to lead fulfilling lives.
  • Safeguarding Advocate: Undertake section 42 Safeguarding enquiries, adhering to Making Safeguarding Personal principles, ensuring the safety and well-being of vulnerable individuals.
  • Mental Capacity Expert: Perform Mental Capacity assessments and make Best Interests Decisions as per the Mental Capacity Act 2005, safeguarding individual rights.
  • Self-Neglect Specialist: Assess individuals who self-neglect, following local multi-agency policies, making a real difference in their lives.

The Ideal Candidate

Essential:

  • Qualified social worker with registration with SWE
  • Significant experience as a Social Worker Strong knowledge of relevant legislation, safeguarding principles, and self-neglect procedures.
  • Commitment to professional development and team support.
  • Access to a vehicle for travel and a driving license 

Desirable:

  • Leadership or supervisory experience.
  • Practice Educator or Best Interests Assessor qualification.
