Job Introduction
An exciting opportuning has arisen for a Senior Practitioner to join our social work service based at Connections Day Service Radstock but work across the Bath and North East Somerset area. You will be working within our Specialist Services across our Learning Disabilities, Autism and Hearing and Vision Team.

You will undertake assessments and reviews with vulnerable adults with an autism diagnosis, adults with learning disabilities, and adults with impaired hearing and vision, their carers, and to work with service users to create care and support plans which meet their needs.
You will work Monday - Friday covering core hours of 9am - 5pm, with the flexibility to work remotely from home to support and maintain a work-life balance. This is a full-time position, there is room for flexibility within this upon discussion.

Main Responsibility
As a Senior Practitioner you will manage a caseload which will include complex cases and will work independently and effectively, under the general supervision of a Team Manager.
Responsibilities will include but are not limited to:
- Provide leadership and assist the Service Manager with a variety of management tasks within the team.
- Play a lead role in supervising other members of the team and supporting social work students
- Chair Multi Agency Risk Management meetings (MARMs) and other complex case meetings
- Undertaking assessments and reviews of people with care and support needs and carers in accordance with the principles of the Care Act 2014
- Creating care and support plans which appropriately meet the needs of people with care and support needs and carers.
- Undertaking Mental Capacity assessments and, where appropriate, complete Best Interests Decisions in accordance with the Mental Capacity Act 2005.
- Undertaking risk assessments and work to reduce risk or engage with the person and MDT in well considered positive risk-taking strategies.
The Ideal Candidate
You will:
- Be a qualified Social Worker
- Registered with Social Work England
- Significant experience working at a Social Worker level or a Senior Practitioner already
- Have a Full UK Driver’s License and access to a car
Ideally you will have had experience working with wider range of adults including people with Autism, Mental Health Conditions or Learning Disabilities.
