Rehab Support Worker

Job Introduction

We are looking for a Rehab Support Worker to join our team! We are based at the Clara Cross Centre Falls, Parkinson’s and Other Related Disorders Clinics held at St Martin's Hospital in Bath.

We are looking for someone to join us part-time for 23.5 hours per week (weekdays only) at a salary of £23,100 (pro-rata) If you are interested we would love to hear from you!

The purpose of this role is to assist in the nursing assessments on medical clinic days, and to carry out initial Occupational Therapy telephone screening assessments and follow-up calls under the supervision of the Occupational Therapists. You may also be required to assist the Physiotherapist with the exercise groups when other staff are off.

Main Responsibility

Key elements of the role are:

  • You will support the Nurse in carrying out a comprehensive Nursing assessment within the Falls and Parkinsons Disease Clinics.
  • Carry out Occupational therapy Screening assessments
  • Carry out cognitive screening  assessments
  • Carry out follow-up phone calls to service users
  • Order equipment
  • Support people to access appropriate information and support, by sign-posting to a range of support services.
  • Complete accurate records in a timely manner, using electronic notes
  • Undertake some administrative tasks as required by the team.
  • You will maintain good communication within the team and with other services eg. GPs, Community rehabilitation teams.
  • You will receive regular supervision and opportunities for training and development.
  • Work in a variety of locations: Clara Cross clinic and when appropriate, support with the assessments in patient’s homes.
  • Support with balance exercise groups when required.
  • Carry out any administrative tasks as required.

The Ideal Candidate

Ideally you will have:

  • experience as a HCA or Support Worker/Therapy Assistance previously.
  • Relevant experience working with older people
  • Experience of working in a team
  • Awareness of health and Social Care

The role is designed to provide support to the Falls Service Nurse, and Occupational Therapy.

At times the role may require the successful applicant to be mobile, visiting several locations across a wide geographical area so the post holder must hold a valid UK driving licence and have access to an appropriate means of transport to undertake the job effectively.
