Position: Reablement Therapy Shift Team Leader - Reablement Therapy Service – band 4

Location: Based at Keynsham Health Centre or The Hollies, Midsomer Norton (cross over between bases expected). Covering cases over the North East Somerset area.

Work Schedule: Working shifts on a set rota pattern including every other weekend and evening shifts when applicable.

Main Responsibility

Key Responsibilities:

  • To apply your extensive community experience, ideally within a reablement or similar team, to set a leading example.
  • Serve as a valuable resource, promptly addressing inquiries from our dedicated Band 3 Reablement Therapy Workers, offering guidance, and nurturing a collaborative work environment.
  • Manage a caseload during your shifts, consistently upholding the highest care standards.
  • Take a proactive role in dynamic casework decisions and management, including providing physical assistance to reablement therapy workers when necessary.
  • Collaborate closely with our coordinator to schedule therapy worker support visits, demonstrating flexibility to reallocate tasks in case of shift disruptions such as sickness or absence.
  • Provide support during Out of main office hours when needed, including covering for sick colleagues, offering extra support when required, or responding to changes in patient health.
  • Ensure the safety of our staff during lone working situations, and monitoring staff sign-in and sign-out procedures.
  • Maintain a valid UK driver's license and access to a car for work purposes, as your responsibilities will span across North East Somerset Reablement.

The Ideal Candidate

Requirements: 

  • Proven experience in a community healthcare setting, ideally within reablement or a related field. 
  • Strong leadership and communication skills
  • Flexibility to work every other weekend and evening shifts (7.5 hours) between 08:00 and 22:00 hours.
  • A valid UK driver's license and access to a car.
