We are looking for a practice nurse to join our dynamic team at Church Lane Surgery. Church Lane Surgery is a forward-thinking service that is committed to delivering excellent care to our patients through innovation, teamwork, and a passion for healthcare delivery. Within the surgery, we are currently undergoing a transformation to improve patient experience and engagement. We have redesigned the way we triage patients by creating a digital front door to our service. We have recently implemented a hub-based MDT triage model, underpinned by best-in-class technology, that is at the heart of helping to transform our care delivery model. Our patients can now submit requests digitally instead of waiting on the phone for hours to speak to a receptionist. The transformation has reduced pressure on the admin team who can now focus on other administrative tasks to support clinicians. Our new triage model has allowed our patients to access our service more rapidly and created an opportunity for clinicians to divert their time to patients with the most urgent requests. The work we have done has had a significantly positive impact on the way patients access our service. We look forward to having you join the transformation!
About Church Lane Surgery
Church Lane Surgery is run by HCRG Care Group on behalf of NHS England. We provide patients with access to high-quality healthcare in Braintree.
The role is based at Church Lane Surgery, in the heart of the historical Saxon town of Braintree, which is roughly 10 miles away from Chelmsford and 16 miles west of Colchester. Church Lane is a vibrant and dynamic surgery offering person centred care to 12,200 patients from the local area. We have a diverse, multi skilled team including paramedics, practice nurses, pharmacists and ANPs, offering a variety of different types of care to our patients as well as sub-specialist services.
We are committed to providing our patients with general practice services and equitable access to health care professionals. We offer a range of services and information as well as advice and treatment for a magnitude of illnesses and minor injuries.
Main Responsibility
- Nursing procedures to include, but not limited to
- Wound management
- Measuring blood pressure & Venepuncture
- Routine injections as prescribed
- Cervical Smears
- Childhood and Adult immunisations, including reading and reviewing Patient Group Directives (PGDs) and Patient Specific Directives (PSDs)
- Well Person Health Checks including Foreign Travel Counselling, NHS Health Checks, Lifestyle and New Patient Checks
- ECG’s
- Spirometry
- Obesity and weight/diet management
- Family Planning
- Chronic disease management and prevention ~ Diabetes, Hypertension, Hypothyroidism, Stroke, Epilepsy, IHD, COPD and Asthma
- Leg ulcer management
- Minor Injuries and Minor Illness
- CVD risk assessments and Q risk interpretation and assessments.
- Home visits for Housebound patients – to include routine checks (eg Diabetic) and flu vaccines.
- Referrals – Podiatry, Counselling, Audiology, etc
- Ensuring all specimens are checked and sent to laboratory as required
- You will be fully conversant in QOF, read coding and uphold strategies to provide a holistic combined healthcare for the patient and demonstrate a willingness to participate in research/audit within the practice team.
- You will be aware of DES/LES policies and apply and produce, alongside the Practice Manager, protocols that reflect the guidelines that are workable in the practice.
- You will be aware of KPIs and work alongside the clinical and managerial team to meet these targets.
- Participate in the education of students of all disciplines.
- To be responsible for the efficient running of the treatment room and equipment and to ensure the clinical areas are always safe for staff and patients.
- To lead on Infection Control in line with guidelines and policies.
- Advise patients and relatives/carers on general health care and ailments with referral to GP’s as necessary.
