Are you an enthusiastic Physiotherapist with a passion for rehabilitation and care of older people? Join our friendly team on Longleat Ward at Warminster Community Hospital and help patients regain independence and achieve their goals.
Working within a supportive multidisciplinary team, you will deliver high-quality rehabilitation and discharge planning, manage your own caseload with guidance from senior colleagues, and support the development of students and therapy assistants.

Main Responsibilities
As a Band 5 Physiotherapist, you will deliver a high-quality physiotherapy service to patients within the community hospital, providing assessment, treatment and rehabilitation to support patients in achieving their optimum level of independence.
Working as part of the multidisciplinary team, you will assess, plan, implement and evaluate patient care programmes, using evidence-based practice to meet the rehabilitation needs of patients. You will adopt a holistic, patient-centred approach, ensuring patients and carers are actively involved in decision-making and goal setting.
You will contribute to effective discharge planning and work collaboratively with colleagues across health and social care to facilitate safe and timely discharges, supporting the reduction of unnecessary inpatient length of stay where appropriate.
You will also provide physiotherapy advice and support to ward staff, patients, carers and partner organisations, promoting effective communication and continuity of care to achieve the best possible outcomes for patients.

• Holds the necessary professional physiotherapist qualification and is registered with the HCPC.
• Has experience working in a relevant Healthcare setting.
• Demonstrates a commitment to high-quality service delivery and continuous improvement.
• Possesses strong communication and interpersonal skills to build positive relationships with service users and colleagues.
• Is able to work effectively within a multi-disciplinary team to deliver integrated solutions.
• A valid UK driving licence and access to a car are essential for this role.

• Additional qualifications or training relevant to the role.
• Experience working in a multi-disciplinary environment.
• Knowledge of industry best practices and innovative service delivery models.
• A desire to further professional development through additional training or qualifications.
