Location: Dartford, Gravesend & Swanley Community Integrated Neighbourhood Teams

About the role
We are looking for an experienced and motivated Specialist Respiratory Physiotherapist to join our Community Respiratory Service. You will play a key role in delivering and developing Pulmonary Rehabilitation programmes and providing expert care for patients with long-term respiratory conditions including COPD, Bronchiectasis, Pulmonary Fibrosis and Asthma.
This is an exciting opportunity to lead service delivery across the community, working as part of an integrated multidisciplinary team to improve outcomes, reduce admissions, and enhance the quality of life for patients and their carers.
What you will be doing
Lead the planning, delivery and evaluation of Pulmonary Rehabilitation programmes in line with national guidance.
Undertake advanced assessments and deliver evidence-based treatments across home, community and clinical settings.
Support patients and carers in self-management to improve confidence and independence.
Work closely with colleagues across primary, secondary and community services to ensure joined-up care.
Supervise and mentor junior staff, students and assistants.
Drive service audits, quality improvement and clinical governance.
What we are looking for
HCPC registered Physiotherapist and CSP member.
Degree in Physiotherapy (or equivalent) with relevant postgraduate training.
Significant post-registration experience in respiratory care and complex case management.
Strong clinical reasoning, leadership and mentoring skills.
Excellent communication and digital competence.
Car driver with access to a vehicle for work purposes.
