We’re looking for a committed and caring Healthy Family Practitioner to join our Lancashire Young People & Healthy Families Service. Based primarily within our Duty Team, you’ll play a vital role in responding to parent/carer enquiries, offering evidence‑based guidance, and supporting the smooth running of our service under the direction of a Health Visitor or Team Leader.

What you'll be doing:

As a key member of our 0–19 team, you’ll:

Duty Responsibilities

  • Provide timely responses to queries via phone or virtual platforms.
    Topics may include feeding, weaning, sleep, toileting, minor ailments, behaviour and general parenting support.
  • Undertake initial assessments and triage, coordinating onward referrals to HVs, HFPs or services such as SALT, physiotherapy, smoking cessation or sexual health.
  • Offer follow‑up calls to monitor family progress and outcomes.
  • Liaise with other professionals as directed by the HV or Team Leader.
  • Recognise, document and escalate safeguarding concerns.
  • Maintain accurate and contemporaneous electronic health records.
  • Support Team Leaders by communicating staff changes, cancellations or sickness reports.
  • Carry out any delegated tasks to support smooth service delivery.

Clinical & Community Work

You will also contribute to wider service delivery by:

  • Completing delegated Healthy Child Programme contacts (3–4 months, 6 months, 1 year, 2–2.5 years and school‑readiness checks).
  • Facilitating Healthy Family Drop‑Ins / Child Health Clinics within local Family Hubs.
  • Providing targeted interventions including behaviour management, infant feeding and sleep support.
  • Helping deliver Bump, Birth & Beyond sessions across the East locality (evenings, 17:30–19:30).
  • Acting as a link to nurseries and early years settings.
  • Participating in CAF/Early Help processes and acting as lead professional when appropriate.

Safeguarding & Risk

  • Identify and escalate safeguarding concerns in line with policy.
  • Participate in risk assessments relating to staff, clients and environments.
  • Follow lone working, infection control and health & safety procedures.
  • Report incidents, accidents and near misses promptly.

What we're looking for:


Essential Qualifications

  • NNEB or equivalent qualification (e.g., BTEC National Diploma in Nursery Nursing).

Experience

  • Experience in health promotion or public health.
  • Experience working within a team environment.
  • An understanding of child protection and safeguarding principles.

Skills & Knowledge

  • Strong understanding of child development and parenting.
  • Flexible, adaptable and able to meet varying service needs.
  • Ability to work autonomously within a multidisciplinary team.
  • Confident in maintaining accurate, detailed and timely records.
  • Able to contribute to audits, quality improvement and research.
  • Skilled in developing and implementing action plans with children and families.
  • Excellent written and verbal communication across agencies and disciplines.
  • IT literate with confidence using digital systems and platforms.
  • Commitment to ongoing professional development.
  • Understanding of clinical governance and safe practice.
