Job Introduction
We are looking for an enthusiastic Healthcare Assistant to join our Falls Prevention Team!
Our Falls Prevention service supports the principles of a 'keeping patients safe in their own home' approach using creative strategies, specialist assessments and bespoke interventions to help patients remain at home and not to attend the acute hospital unnecessarily. We often receive referrals directly from patients, carers or relative where patients have sustained non-injurious falls where the team can assess and assist to prevent further injury and long lays for patients on the floor.
These assessments inform a vast array of interventions, including use of the Hoverjack lifting device, which result in excellent outcomes for our patients.
Our Falls Prevention Service runs alongside our Rapid Response and Intermediate Care Service based at Livingstone Community Hospital, Dartford. This role is a permanent position working 22.5 hours a week.

Main Responsibility
As a Healthcare Assistant for the Falls Prevention team you will:
- Support with running the Strength and Balance class on every Thursday
- Carry out home visits to support with exercises, review equipment and complete Wellbeing assessments
- Provide information, teaching and instruction to patients, relatives, carers, other disciplines, and agencies.
- Participate in and support qualified team members in health promotion activities
- Provide clerical and administrative support to the team
- Report any changes in condition of the patient to the Lead Therapist or in their absence the designated therapist in charge of the caseload.
- Work with Physios and advanced Rehab assistants within the team to actively support patient rehabilitation programmes, according to care plans created by therapists

The Ideal Candidate
The ideal candidate will have strong communication and organisation skills and the ability to recognise own abilities and work within sphere of competence.
We are looking for someone who is a friendly, approachable team player who can work flexibly and will ensure they deliver positive experiences to patients at every stage of their healthcare journey.
It is desirable that you have experience dealing with the general public within a healthcare setting and ideally experience working in a healthcare setting and in particular community care. Full training will be provided when in post.
