At HCRG care group we are dedicated to transforming healthcare. As a Health Visitor you will devise and implement personalised care plans for each child based on a needs assessment. You will cover the localities of Radstock, Midsomer Norton, Paulton and the surrounding areas.
You will join a welcoming and supportive team of Band 6 SCPHN Health Visitors, Band 5 Staff Nurses and Early Years Community Practitioners working to deliver Universal Children’s Services! We work in line with the Healthy Child Programme to improve health, reduce inequalities and safeguard children & young people.

Our Somer Valley is located across the towns of Radstock, Midsommer Norton and Paulton, with easy access to nearby shops and cafes near the base. Our service hours are typically 9am to 5pm Monday to Friday with opportunities for flexibility upon discussion.

Main Responsibility
As a Health Visitor you will:
- Devise and implement individualised care plans for each child based on a needs assessment
- Identify and assess health, growth and development in the 0-5 age range (0-19 Pathway) to advise and refer appropriately
- To follow the guidance of the Unicef Baby Friendly Initiative across the organisation .
- Provide information and education in support of Childhood Immunisations.
- Participate in effective partnership working with other agencies, including Early Help Assessment (EHA) of children’s needs
- Actively involve the child / parent / carer / family in the planning and implementation of any proposed programmes.
- Identify children at risk and follow B&NES safeguarding policies and procedures.
- Provide information and education in support of Childhood Immunisations.

The Ideal Candidate
Our Ideal Candidate will:
- Hold NMC registration
- Be a qualified SCPHN Health Visitor
- Have a driving license and access to a car
- Experience working as a health visitor or in a health visiting or children’s nursing team previously
We also welcome applications from newly qualified SCPHN Health Visitor students as well as students currently not yet completed their course.
