Main Responsibility

As a Health Improvement Practitioner within HCRG Care Group you’ll join a highly skilled team who are focused on service  excellence.

Your days will be rewarding and varied and include:

  • Be a key member of the Stop Smoking Team based within the Wellness Service and the Community Wellbeing Hub.
  • To support and develop the stop smoking by helping service users to stop or reduce smoking focusing on areas of high depravation and need. 
  • To be able to train other health professionals to deliver smoking cessation advice and support.  
  • To help ensure a public health focus on the wider issues of tobacco control. 
  • Support/develop, facilitate, and evaluate services and training courses for general public and a range of health promoters in partnership with colleagues.
  • Raise awareness of the Wellbeing Service including the Community Wellbeing Hub and disseminate information to relevant organisations eg Council, Charities and service users.  
  • To be able to work when needed on delivery of other well-being services. 

The Ideal Candidate

We are looking for someone who can bring their knowledge and personality to our team. You must have experience of working with service users to make a behaviour change and have a real interest in smoking cessation and the wider public health agenda on smoking and public health. 

You will need to have an adaptable and flexible skills base and be able to work well with a knowledgeable team. 

You will need to be flexible with working hours to meet the needs of the service. To include occasional evenings.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel, Teams.
