GP with Extended Role (GPwER) - Frailty

Job Introduction

We are seeking a dedicated GP with Extended Role (GPwER) to join our clinical team and provide expert medical care!

Your role will primarily be based through our Hospital at Home service, but with cross over involvement within our inpatient units. This dual-setting role focuses mainly on older patients with multiple health conditions and those with advanced clinical needs.

You will play a vital role in bridging the gap between acute hospital care and community-based services, contributing to an evolving Hospital at Home service.

You will be based within the inpatient units at St Martin’s Hospital, Bath or Paulton Hospital or within patients’ homes across Bath and North East Somerset. Service hours run from 8am-8pm, 7 days a week.

Main Responsibility

As a GPwER or Specialist Doctor, you will:

  • Provide medical care to patients in inpatient community units at St Martin’s Hospital, Bath, and Paulton Hospital, as well as patients' homes across BaNES.
  • Work within multidisciplinary teams to improve patient outcomes and ensure safe, effective care with optimized lengths of stay.
  • Collaborate closely with the Royal United Hospital Bath (RUH) Hospital at Home team to provide acute multidisciplinary care in the home setting, focusing on admission avoidance and supporting early discharge from the acute hospital setting.
  • Contribute to the strategic development of clinical practices within our services and strive for excellence in care delivery.
  • Support and lead clinical teams in providing specialist interventions, ensuring continuity of care across various health and social care interfaces.
  • Act as a professional role model and provide leadership and consultancy for the coordination of urgent healthcare needs, particularly for those traditionally requiring hospitalization.
  • Deliver evidence-based clinical practice, including assessment, diagnosis, treatment planning, and timely response to changing clinical needs.
  • Participate in audit and research activities to drive continuous improvement and contribute to the development of new knowledge and practices.
  • Maintain active registration as a medical prescriber, managing medication needs for patients in Hospital at Home and preparing for discharge.

The Ideal Candidate

We are looking for a GPwSi or a Specialist Doctor who is:

  • Registered with the GMC.
  • Hold a CCT as a General Practitioner and relevant qualifications and experience for older/frail adult care.
  • There will be a requirement to travel across the region and at times to go to meetings and training in London or elsewhere in the UK - Full UK driving licence and access to a vehicle will be required.
  • Passionate about providing high-quality care to the local community.
  • Experienced in managing frail elderly patients and those with advanced clinical needs.
  • Skilled in working within a multidisciplinary team across primary, secondary, acute, and social care settings.
  • Able to lead and develop innovative healthcare practices and contribute to continuous service improvement.
  • Committed to professional development and supporting a positive learning environment for colleagues and students.
