District Nurse - Band 6 / Senior Community Nurse

Job Introduction

An exciting opportunity has arisen for a District Nurse / Community Nurse Band 6 to join our Farnham Community Nursing Team to help us continue to change things for the better

The Farnham Community Nursing team is based at Farnham Hospital and covers Farnham and surrounding areas. They are a highly supportive team who are happy to develop an aspiring Band 5 nurse into a Band 6.

They are also able to offer flexible working to provide a good work life balance.

The community nursing team covers 8am to 8pm, 365 days a year. The shifts include a mixture of 8am to 4pm,  9am to 5pm and 12pm to 8pm. and 1 weekend in 3. We are also happy to consider part time hours.

Main Responsibility

Within this role you will:

  • Provide leadership and management to your team.
  • Have responsibility for your caseload and have skills and competencies to provide excellent patient care in their homes.
  • Undertake full nursing assessments of patients, including those with multiple co-morbidities and complex health and social needs.
  • Be responsible for agreeing the patient’s care plan, liaising with others and adapting the care and treatment delivered according to need and patient choice.
  • Work with the Integrated care system and MDT to provide holistic care for your patients.
  • Responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
  • implement, monitor and maintain high standards of care delivery at all times.
  • Care for patients and their families in the way that you believe is best
  • Deliver our high service standards, sharing best practice, and actively improving the way we work.

The Ideal Candidate

You will be a NMC/1st level registered Nurse preferably some community experience. You will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that you should have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey.  You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages and have working knowledge of an i pad which is used for mobile working.
