Job Introduction
We have an exciting developmental opportunity for an enthusiastic Band 6 Registered General Nurse wanting to progress their skills within the specialism of Diabetes nursing. Upon completion of the competencies, the role will transition into a Band 7 post.
As a Diabetes Specialist Nurse you will be working with our friendly, enthusiastic and national award winning team, as part of the NHS system in Surrey and Northeast Hampshire.
Within this role there is a great deal of variety from face-to-face contacts with patients and carers in clinic and home settings, to provision of education in groups to both patients and healthcare professionals. You will manage your own caseload and work independently with a wider support function. The Diabetes specialist team works closely with both general practice healthcare professionals, community nursing teams, social services and the hospital diabetes specialist team aiming to provide an integrated holistic approach of care to individuals.
This is a flexible full time opportunity working Monday to Friday 9am – 5pm with the flexibility to work one day a week from home to complete virtual consultations and complete your admin tasks.

Main Responsibility
In this role, you will (but not be limited to):
- Promote the health and wellbeing of people with diabetes
- Provide a link between GP and consultant led specialist teams
- Deliver education and support to people with diabetes and those who care for them
- Be responsible for managing a defined caseload of patients, using theoretical knowledge and high levels of practical experience to formulate a specialised programme of care.
- Accept referrals from anyone involved in caring for people with diabetes and from patients themselves
- Provide education and support to Health Care Professionals

The Ideal Candidate
Our ideal candidate will:
- Be a Registered General Nurse with a minimum of 3 years post graduate nursing experience
- Have evidence of managing people with diabetes whether that be in the community or in a hospital setting
- Have the confidence using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel, Teams
- Hold full UK driving licence
