Job Introduction
You’ll work closely with our Continence Specialist Nurses as part of a close-knit, community-based continence team delivering high-quality care across Swale.
Your role will involve supporting continence assessments, reviewing patients, providing education and advice, and assisting with ongoing care plans for patients with bladder and bowel conditions. The role includes a blend of clinical, office and community working, allowing you to build strong patient relationships and gain valuable specialist experience within continence care.

Main Responsibilities
As a Band 4 Continence Assessor, you will:
- Support the delivery of continence assessments, reviews and follow-up care under the guidance of registered clinicians
- Provide advice and education to patients and carers on bladder and bowel health management
- Assist in developing and implementing individualised care plans
- Work collaboratively with community nursing teams, GPs, AHPs and wider healthcare professionals to support coordinated care
- Monitor patient progress and escalate concerns appropriately to senior clinicians
- Maintain accurate and timely clinical records in line with organisational standards
- Support stock management and continence product provision where required
- Participate in training, supervision and ongoing development to enhance your clinical knowledge and skills
- Contribute to service improvement initiatives and promote high standards of patient-centred care
The Ideal Candidate
We’re looking for someone who:
- Has experience working within a healthcare or community care setting
- Has a genuine interest in continence care, long-term condition management or community services
- Holds a full UK driving licence and has access to a vehicle for work
- Demonstrates strong communication and interpersonal skills
- Is compassionate, organised and able to work independently as well as part of a team
- Has a commitment to delivering high-quality, patient-centred care
- Holds a relevant NVQ Level 3 qualification in Health and Social Care or equivalent experience (desirable)
