Job Introduction
We’re looking for a passionate and dedicated Band 6 Team Sister to join our friendly Community Nursing team based in Gravesend, North Kent, and help us continue to improve the way we deliver care across the community.
Gravesham Community Hospital is located just a short walk from St George’s Shopping Centre, surrounded by local cafés and restaurants, making it an ideal and convenient base.
You’ll be supported by a committed Learning and Development team, with access to in-house eLearning, workshops, and courses to ensure you meet all required competencies. We work closely with Universities and Health Education England to support your professional growth and career progression.
When you join, you’ll receive a full induction and ongoing support from managers, colleagues, and our TLE department to help you settle into your role. Regular team meetings and weekly one-to-ones will ensure you’re supported and encouraged to thrive within the team.
Our Community Nursing team operates 7 days a week, from 08:00 – 20:00, providing flexible and responsive care to patients in their own homes.
A full UK driving licence and access to a vehicle for work purposes is essential.
Main Responsibilities
As a Community Nurse, you will:
- Deliver high-quality, compassionate care to patients in their homes and community settings.
- Use your clinical expertise, strong communication skills, and organisational ability to ensure safe, effective, and person-centred care.
- Work collaboratively with colleagues across the multidisciplinary team to support patients and their families through their healthcare journey.
- Contribute to continuous improvement within the service by sharing ideas, best practice, and feedback.
- Maintain accurate and up-to-date patient records in line with professional and organisational standards.

The Ideal Candidate
- NMC/1st level registered Nurse
- Ability to work both independently and as part of a multidisciplinary team
- Excellent communication and organisational skills
- Flexible, proactive, and committed to delivering exceptional patient care
- Confident using IT systems, with working knowledge of Microsoft Office (Outlook, Word, Excel)
- Full UK driving licence and access to a vehicle for work purposes (essential)
