Job Introduction
We’re looking for a caring, compassionate and dedicated Community Support Worker to join our team.
The role of the community support worker is to work within a multi-professional team in a community setting, providing, support, care, and treatment to individuals, within guidelines and protocols identified for the role.
The community support worker will provide support with guidance from nurse and/or therapist to deliver care plans to patients in a community setting. Depending on experience and skill mix, you will carry out nursing or therapy related activity.
As our services transform, there may be times you will be expected to support both nursing and therapy in the delivery of care, within an appropriate scope or practice and following relevant on-the-job training.
This is a full-time role working 37.5 hours per week.

Main Responsibilities:
- Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
- To keep the patient at the centre of their care, ensuring patient and carer participation in decision making.
- Responsibility for, and supporting, the day-to-day care of patients, including monitoring.• To communicate with individuals about routine and daily activities, this may include overcoming difficulties (e.g. hearing/language).
- To work within a multi-professional team and to feed back service user progress, liaising with colleagues, families, carers and other agencies.

Person Specification -
Essential
- Good general education (English/Maths Grade A-C) and Level 2 NVQ/QCF in Health and Social Care or equivalent.
- Level 2 NVQ/QCF in Health and social care or equivalent.
- Care Certificate – or willingness to achieve.
- Experience of working under supervision.
- Experience of supporting other workers.
- Ability to work as a team member.
- Delivering patient focused care.
- Ability to recognise own limitations.
- Typing/data inputting skills.
- Physical skills to manually handle patients and use appropriate lifting aids.
- Experience in providing care / delivering rehab or exercise.
- Driven to achieve.
- Shows commitment to ongoing personal and professional development.
- Good time management skills.
- Full UK driving licence and use of a car during working hours.
Desirable:
- Experience of community working.
- Experience in health care.xperience of work in an acute or urgent health care setting.
- Experience of working in rehabilitation.
- Basic wound care knowledge and skills.
