We are looking for a caring, motivated, and enthusiastic Band 3 Community Rehabilitation Assistant to join our Intermediate Care Team.
About the Role
Working across Surrey Heath and Farnham, you will support people in their own homes following illness, injury, hospital admission, or a decline in function. As part of a multidisciplinary team, you will help patients regain confidence, improve their independence, and achieve their rehabilitation goals, enabling them to remain safely in the place they call home.
This is a rewarding role that combines care and rehabilitation, placing the patient at the centre of everything we do. You will work alongside Physiotherapists, Occupational Therapists, Nurses, and other healthcare professionals to deliver high-quality, person-centred care.

- Deliver person-centred care and rehabilitation in accordance with individualised rehabilitation plans developed by the therapy team in partnership with patients and their families.
- Support patients with activities of daily living, including washing, dressing, personal care, simple meal preparation, and medication support where appropriate and within agreed competencies.
- Assist patients with exercise programmes and rehabilitation activities prescribed by Therapists, encouraging participation, confidence, and progression towards agreed goals.
- Empower individuals to do as much as possible for themselves, promoting independence, dignity, and wellbeing at every opportunity.
- Monitor and document patient progress using a tablet, ensuring records are maintained accurately and contemporaneously while working in the community.
- Recognise and report any changes in a patient's physical, emotional, cognitive, or functional wellbeing.
- Work collaboratively with Nurses, Physiotherapists, Occupational Therapists, and other healthcare professionals to deliver safe, effective, and coordinated care.
- Contribute to the delivery of safe, effective, compassionate care in line with organisational policies, procedures, delegated responsibilities, and competency frameworks.
- Undertake administrative duties required to support patient care and the smooth running of the service.
- Maintain and develop the knowledge, skills, and behaviours required to provide high-quality care that responds to the changing needs of patients and the wider healthcare system.
