Job Introduction
We are looking for an accredited Advanced Clinical Practitioner (Nurse or AHP) who will take a lead in the advanced assessment, management and treatment of patients in community settings with particular focus on those referred for the 2 hour Urgent Community Response (UCR).
The role will involve undertaking visits to patients living at home or in care homes across Bath and North East Somerset as part of urgent community response UCR.

Main Responsibility
Working as part of a multidisciplinary team and across the primary, secondary, acute and social care interfaces in BaNES, you will be responsible for coordinating the provision of specialist interventions to meet the urgent health care needs of people at risk of hospitalisation or those requiring facilitated timely discharge from emergency departments.
You will gain a systematic patient history, perform assessment and physical examination and diagnose before making a management and treatment plan in partnership with the patient and wider team. The team support UCR are comprised of Advanced Clinical Practitioners, Community Nurses, Community Matrons, Therapists and Social Workers. The UCR programme has been established and a key focus for this role is further expand the 9 clinical pathways and to increase referrals into the service to reduce conveyances to the emergency department.
The Ideal Candidate
We are looking for an Advanced Clinical Practitioner (Nurse or AHP) who has completed an Advanced Practice programme (minimum 120 credits at Master's level).
You must demonstrate excellent clinical, communication and organisational skills and enjoy working in a new team and be an innovative Advanced Practitioner clinician.
A strong clinical background in older person's care and frailty is important.
