Community Diabetes Specialist Nurse (Development Post) Band 6

Full time post, Monday to Friday 9am- 5pm (hours negotiable)

Are you a passionate and committed Registered General Nurse wanting to work within the specialist field of Diabetes in the community setting? Are you looking to join an NHS Health and Social Care partner which is striving to make positive changes? 

We are looking for a RGN Band 6 applicant wanting to develop their skills within the specialism of diabetes nursing. You’ll be working with our friendly, enthusiastic, and dynamic team as part of the NHS system in Surrey and Northeast Hampshire.

Main Responsibility

In this role, you will (but not be limited to):

  • Promote the health and wellbeing of people with diabetes
  • Provide a link between GP and consultant led specialist teams
  • Provide education and support to people with diabetes and those who care for them 
  • Be responsible for managing a defined caseload of patients, using theoretical knowledge and high levels of practical experience to formulate a specialised programme of care.
  • Accept referrals from anyone involved in caring for people with diabetes and from patients themselves
  • Provide education and support to Health Care Professionals

The Ideal Candidate

A Registered General Nurse with a minimum of 3 years post graduate nursing experience.

Evidence of managing people with diabetes and completion of a qualification in diabetes is desirable.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel, Teams
