Clinical Triage Practitioner
An exciting opportunity has arisen for a Clinical Triage Practitioner to support the delivery of Children's Specialist Services across Bath & North East Somerset, Swindon and Wiltshire.
The post holder will undertake clinical triage of referrals, including urgent referrals, ensuring timely assessment, decision-making and communication of outcomes. Working as part of a multidisciplinary team, the successful candidate will support complex referral discussions, provide clinical advice and contribute to effective care coordination across services.
The role requires close partnership working with healthcare professionals, education services, voluntary sector organisations and families to ensure children and young people receive appropriate, safe and effective support through a coordinated and integrated care pathway.
Location: To be agreed within Bath & North East Somerset, Swindon or Wiltshire, with hybrid home working available. Travel across the BSW locality may be required to support service delivery and partnership working.
Hours available: 30 hours per week.
The service operates on a rota basis, and the successful candidate will be required to work either:
- 8:00am to 4:00pm, or
- 10:00am to 6:00pm

Main Duties of the Role
- Undertake clinical triage of referrals into Children's Specialist Services and School Aged Health Services in accordance with agreed pathways and timescales.
- Support the Clinical Triage Lead in the delivery of multidisciplinary triage for complex and multiple-need referrals.
- Provide specialist clinical advice and support to Care Coordination and Single Point of Access functions.
- Ensure children, young people and families are directed to appropriate services through effective care navigation.
- Communicate complex, sensitive and potentially distressing information to families and professionals as required.
- Work collaboratively with health, education, social care and third-sector organisations to ensure coordinated care delivery.
- Identify, escalate and manage safeguarding concerns, clinical risks and pathway issues.
- Contribute to service development, governance processes and implementation of standard operating procedures.
Person Specification
Essential Criteria
- Registered Nurse or Allied Health Professional with current NMC/HCPC registration.
- Evidence of continuing professional development.
- Evidence of specialist clinical knowledge relevant to the role.
- Minimum 3 years' post-registration experience.
- Knowledge of current health and social care policies.
- Knowledge of safeguarding and child protection procedures.
- Experience of child and family-centred care.
- Demonstrable clinical engagement and assessment skills.
- Experience working with partner agencies and multidisciplinary teams.
- Ability to work across professional and organisational boundaries.
- Supervisory and reflective practice skills.
- Ability to travel throughout Wiltshire, Bath & North East Somerset and Swindon.
Desirable Criteria
- Experience working in a community setting.
- Experience of clinical triage and care coordination.
- Knowledge of Education, Health and Care Needs Assessment processes.
- Experience supporting complex or multi-agency cases.
- Service evaluation, audit or quality improvement experience.
- Teaching or mentorship experience.
