Clinical Pharmacist - HMP Chelmsford

Job Introduction

An opportunity has arisen for an enthusiastic and dynamic pharmacist to join our forward-thinking healthcare service within HMP Chelmsford.

The main focus of the work will be to provide a dispensary based pharmacy service and working closely with the Lead Pharmacist and pharmacy technicians on providing medicines management support to a number of different wings within the prison.

You will be based within the in-house pharmacy team, ensuring safe and smooth running of the dispensary i.e.

  • supply of medications to wings, depending on the business needs
  • ensuring the safe and secure transport of medicines within the prison
  • ensuring adherence to Controlled Drugs regulations
  • providing professional supervision of the pharmacy technicians

With appropriate training you will be providing medication review services to patients via clinics, deliver pharmaceutical care plans, and improve the quality of patient care and medicines optimisation as assigned by the Lead Pharmacist. This is an exciting job with potential opportunities in medicines optimisation, prescriber training and further long-term condition management.

Please note that, in any case, the healthcare staff will not be expected to be responsible for or involved with prisoners' restraints.

Within the prison pharmacy you will see everything you would in a community pharmacy and a hospital under one roof and you will work closely with other healthcare colleagues across teams to ensure the patients get equivalent care to those in the community.

Main Responsibility

You'll be responsible for the provision and delivery of an integrated, safe, legal, high-quality prison pharmaceutical service.

The role also involves responsibility for providing daily expert advice on pharmaceutical matters and ensuring that a safe but modern medicines management service is provided to prisoners.

You will also be involved with:

  • Undertaking medication use reviews with patients and produce recommendations for other clinical pharmacist, nurses and/or GP on prescribing and monitoring
  • Reviewing the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation)
  • Managing caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence
  • Managing the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review
  • To take professional responsibility when clinically screening prescriptions to ensure the accuracy, legality, safety and efficacy of prescribed medication, including liaising with prison GPs and nurses to resolve any queries arising from the screening process
  • Administer medications on the wing, depending on the business needs

The Ideal Candidate

Essential Experience / Qualifications

  • You will be a fully qualified pharmacist registered with the GPhC
  • Must be confident and capable of autonomous practice and able to work independently

Desirable Experience

  • Experience in a Prison environment
  • Experience of working as part of a multi-disciplinary team
