Job Introduction

Are you looking for an exciting new opportunity as a Care Coordinator? HCRG Care Group are looking for someone to join our team on a 1 year fixed term basis, acting as a dedicated resource to support the timely facilitation of discharge from Acute and Community Hospitals into the community. You will develop relationships with the service User and their families, empowering them to make decisions around their care journey, while supporting a ‘Home is Best’ approach and promoting independence.  

Working as part of the Care Coordination Centre at Peasedown St John the post holder will in reach into Acute and Community Hospitals at the point of referral. Working as part of a multidisciplinary team you will proactively coordinate tailored discharges by acting as a single point of contact for the service user, family, and professionals from multiple providers, including close liaison with Care Providers. Following discharge from hospital you will continue to support the service user throughout their journey, where required visiting the Service User in their own home to expediate referrals to community services, including therapy services, Social Care, Wellbeing and Voluntary services. Providing case management to an allocated caseload will allow regular review of the Service User’s short and long-term needs allowing completion of their care journey within 4 weeks of discharge.

Main Responsibility

As part of the HCRG Care Group Care Coordination Team you will report to the Therapy Clinical Lead. The team consists of Care Coordinators, Triage Clinicians, Clinical Leads from a Flow, Nursing and Therapy discipline, Wellbeing advisors, Social Workers and Social Care advisors. In your role as a Care Coordinator, you will:

  • Ensure people using our services and families are aware of the discharge process and expected timeframe for discharge providing and overview of reasons discharges can be delayed and the role of discharges services in supporting their journey home.
  • Ensure onward referrals to commmunity services are actioned in a timely manner, following standard operating procedures.
  • Follow peoples journey from referral to discharge from the service and as required onto their long term service provision
  • Coordinate MDT meetings with the support of clinical leads to ensure any blocks to patients care are prevented facilitating a smooth care journey escalting to Head of Care Coordination and partners for advice and suport.
  • Facilitate and promote 7 day discharges from hospital working closely with care providers.

The Ideal Candidate

  • Have excellent communication skills, written, verbal and non-verbal. Adapting their approach depending on the audience to build authentic, proactive working relationships.
  • Be highly organised, to enable effective case management and review, allowing reduction in care provision and early identification of any longer-term care needs.
  • Be able to work in a time critical manner, to enable discharge within 24-48 hours from the point of referral.
  • Demonstrate kind and compassionate care to all Service Users and their families.
  • Have the ability to identify any blockages in service provision and escalate these to the Clinical Lead.
  • Have Excellent IT skills, with the ability and desire to learn new Clinical System platforms, such as Liquid Logic and SystmOne.
  • Experience in Acute or Community Care is welcomed
