Are you a Care Co-Ordinator looking for an exciting new opportunity?

CGH Partnership are looking for a Care Co-Ordinator for 37.5 hours per week.

Main Responsibility

Key duties will include:

  • To line manage the Care Co-ordinator team and oversee the care co-ordination function within the practice. To promote an efficient and a patient focused approach. Oversee and coordinate the care and support of the patients of the teams. 
  • You will allocate work according to demand and ensuring team members are working to their competencies and you will be ensuring that KPIs are met in terms of the turnaround time targets as well as being proactive in recognising potential issues with the view of resolving them quickly.
  • The Care Co-Ordinator Lead will sift through the workflow to ensure that is all appropriate for the clinical team to undertake.
  • The post holder will work closely with the social prescribers for any social issues that patients present with and they will also attend multidisciplinary meetings on a regular basis.
  • You will carry out administrative duties on behalf of the clinical team.

The Ideal Candidate

Interested? What you will need to be considered:

  • Experience of administrative duties.
  • Experience of working with healthcare and pervious experience in the NHS or social care.
  • Experience in use of GP clinical systems and knowledge/familiarity with medical terminology.
  • Able to demonstrate a clear understanding of working with confidential information.
  • Ability to work in a busy and demanding environment.
