Job Introduction
We have an exciting opportunity for an experienced and motivated Clinical Lead Nurse to join our Urgent Care Response (UCR) team covering the Swale locality.
This is a key leadership role responsible for the clinical oversight, operational coordination and ongoing development of the UCR service. You will support the delivery of high-quality, responsive care to patients in their own homes, helping to prevent unnecessary hospital admissions and facilitating timely interventions for those with urgent healthcare needs.
This is a full-time position working 37.5 hours per week, ideally 08:00 to 16:00 Monday to Friday, with occasional weekend working as required.
Main Responsibilities
- Provide clinical leadership and operational oversight for the UCR team.
- Manage the day-to-day delivery of the service, ensuring safe, effective and patient-centred care.
- Undertake advanced assessment, planning, implementation and evaluation of care for patients with complex needs.
- Act as an autonomous practitioner, making sound clinical decisions and delegating appropriately.
- Support, supervise and develop team members, promoting high standards of practice.
- Ensure all documentation is completed in line with NMC standards and HCRG Care Group policies.
- Monitor service performance and contribute to continuous improvement initiatives.
- Respond to urgent issues involving both patients and staff, providing effective leadership during challenging situations.

The Ideal Candidate
The ideal candidate will have:
- Current registration with the Nursing and Midwifery Council (NMC)
- Evidence of continuing professional development and education at degree level or equivalent
- Leadership or management qualification, or willingness to work towards one
- Teaching and assessing qualification or equivalent experience
- Significant post-registration experience, including community nursing experience
- Experience of leading teams and managing services
- Experience of caring for patients with complex and long-term conditions
- Strong clinical decision-making and organisational skills
- A full UK driving licence and access to a vehicle for work purposes
