Job Introduction
We are excited to offer an opportunity for a committed and dynamic Registered Nurse to join our Virtual Ward team as a Team Sister/Charge Nurse. In this leadership role, you will manage a defined caseload of patients receiving remote and in-person support, ensuring safe, efficient, and high-quality care tailored to individual health and social needs.
You will work closely with the Clinical Lead to support triage, caseload delegation, and the supervision and development of nursing staff. This is a varied and rewarding position where you'll make a real difference by helping patients with long-term conditions, frailty, or recovering from illness to remain at home and avoid unnecessary hospital admissions.
This role is well-suited to an experienced Band 5 or 6 nurse looking to step into a leadership post, with support for ongoing professional development and mentorship.
Main Responsibility
Lead and coordinate Virtual Ward patient care and oversee clinical interventions for long-term conditions, frailty, and complex needs
Provide leadership and supervision to nursing colleagues, supporting professional growth, rota planning, and first-line management
Conduct clinical triage of referrals and support safe and timely caseload allocation
Promote evidence-based care and patient safety, contributing to quality improvement and incident management processes
Deliver expert clinical interventions including IV therapy, symptom management, and end-of-life care (training available)
Liaise with GPs, MDT professionals, and external agencies to ensure integrated care planning
Ensure accurate and timely clinical record-keeping in line with governance and data protection standards
Participate in appraisals, training, audits, and the implementation of service developments
The Ideal Candidate
Essential:
NMC Registered Nurse with 3+ years of experience in community, hospital, or integrated settings
Experience managing long-term conditions and complex care needs
Strong leadership and mentorship abilities
Excellent interpersonal and communication skills
Ability to manage a caseload, prioritise tasks and work autonomously
Sound knowledge of clinical governance, safeguarding, and quality standards
Competent with IT systems and electronic record-keeping
Current UK driving licence and access to a car for work
Desirable:
Teaching and Assessing qualification (ENB 998 or equivalent)
Independent Prescribing qualification or willingness to undertake
Experience in a clinical supervisor or first-line manager role
Experience of delivering care through a Virtual Ward or community service model
