Job Introduction
We have an exciting opportunity for a dedicated and passionate Band 6 Team Sister to join our new virtual ward service, based at Sheppey Community Hospital and covering the SWALE area.
This is a great opportunity to be a key driver in supporting patients to recover from or manage their illness and enable them to stay healthy and well for longer. You will have the responsibility of managing a defined Virtual Ward caseload and will utilise your clinical skills, leadership skills and expertise to assess, triage, plan, delegate, organise and evaluate complex packages of health and social needs tailored to individuals and ensure that patients receive the right care at the right time by the right person.
Main Responsibility
- To provide caseload leadership and management that demonstrates quality, safety and productivity in line with commissioning intentions.
- Act as a role model for the delivery of high standards of care that complies with NMC codes of conduct and Trust policies.
- Document all patient interventions contemporaneously onto electronic record ensuring accuracy and the inclusion of all appropriate assessments.
- Ensure correct use of confidential and sensitive information in line with Caldicott.

The Ideal Candidate
The ideal candidate will be / have:
- Current NMC Registration
- Teaching and Assessing NMC approved qualification
- Evidence of relevant post registration specialist professional development
- 3 years’ experience as a registered nurse
- Experience in a variety of settings including hospital and community
- Experience of managing a range of long-term conditions
As this is a community based role, a full UK driving licence and access to a vehicle for work purposes is essential.
