Job Introduction
We have an exciting opportunity for a part time Band 6 Physiotherapist to join our Urgent Care Response team!
The Urgent Care team provides a two-hour assessment and short-term intervention to prevent hospital admissions. This role involves comprehensive assessment and intervention under the guidance of clinical specialists, offering expert advice and managing complex patients within a virtual multidisciplinary team (MDT). After an initial assessment, a care plan is developed in consultation with the service user, who is encouraged to actively participate.
Our virtual MDT is made up of various healthcare professionals, collaborations with family, health and social care colleagues, and voluntary organisations to provide person-centred care.
This is a part time role working 33 hours across the week (fewer hours will be considered). You will be based primarily out of the Care Coordination Centre in Peasdown-St-John, the role covers the BaNES area and requires mobile and home working. Regular supervision and CPD are provided, with the possibility of rotational work with the acute trust or wider reablement teams.
The service operates from 8am to 8pm, seven days a week with the expectation of working 1 weekend per month and occasional bank holidays.
Main Responsibility
As a Band 6 Physiotherapist in the Urgent Care team you will:
- Conduct holistic assessments, implementing, and evaluating care programs for acutely and chronically unwell patients.
- Manage a caseload autonomously within the UCR pathways, using clinical reasoning to plan and evaluate patient-centred rehabilitation programs.
- Promote high-quality, harm-free care, and maintaining patient safety at home.
- Work within a multidisciplinary team to develop and deliver specialised services, sharing skills and knowledge.
- Conduct skilled assessments to formulate individualised treatment plans using evidence-based practices.
- Encourage independence and providing advice, education, and training to patients and carers.

The Ideal Candidate
- Degree in Physiotherapy/Occupational Therapy and registered with HCPC.
- Previous post-qualification experience in a health and social care environment within the physiotherapy/occupational therapy discipline.
- Community experience
- Depth of knowledge in physiotherapy/occupational therapy principles and practices.
- Ability to interpret and explain technical information to various audiences.
- Proven problem-solving skills and sound decision-making ability.
- Car driver with access to a vehicle and business insurance for independent travel within Bath and North East Somerset.
