As an occupational therapist working within the Reablement service you will work with people in their own place of residence to maximise and maintain independent function. You will be competent in carrying out occupational therapy assessments and identifying person centred goals.
You will work collaboratively with our team of Physiotherapists, OTs, Nurses and Rehab Assistants, to deliver high quality care to patients in the community by assessing their needs and implementing a treatment plan to encourage their independent living and prevent hospital admissions.
Main Responsibility
Following an initial assessment and in consultation with the service user, you will devise an appropriate care plan and rehabilitation programme. Service users are encouraged to take an active part in the development of their plans and are supported to make informed choices regarding their care.
Working within the Reablement team team, you will maximise and maintain independence through the provision of rehabilitation and support.The service supports facilitation of hospital discharge, avoidance of admission, planned therapeutic intervention, stabilising care plans, and identifying ongoing care needs.
The Ideal Candidate
You will have a Degree qualification in Occupational Therapy and be registered with HCPC
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
