Job Introduction
We are looking for a Band 6 Nurse to join our Integrated Urgent Community Response Team in West Lancashire. The team responds to urgent referrals into the service from GP/111/Care Homes and other HCPs for patients to be seen at home.
This is a Full Time, Band 6 post, working 37.5 hours a week with Agenda for Change terms and conditions. The service is open 365 days a year and is operational between the hours of 8am – 8pm.

Main Responsibility
Responsibilities include but are not limited to:
- Respond to urgent requests for patient home visits where the focus of the visit is admission avoidance. Provide timely, high quality, person centred clinical care to patients
- Have the necessary knowledge, skills and training to recognise emergency clinical situations such as anaphylaxis, cardiac and respiratory deterioration, to act appropriately knowing when to seek assistance
- To provide high quality palliative care to patients and their families, as required
- Maintain effective communication both within and outside of the clinical team to ensure that patient needs are met by the most appropriate agency
- Undertake mandatory training in line with organisational policy, and attend updates and study days, as appropriate to the post

The Ideal Candidate
- Be a registered nurse with NMC registration and a passion for providing high quality holistic care to patients.
- Ability to act independently as well as part of the multi-disciplinary team.
- Have the ability to problem solve and make clinical decisions, confidently.
- Have a valid UK driving licence, with access to a car for work purposes.
