Band 6 Diabetes Specialist Nurse

Job Introduction

We are looking for a Band 6 Diabetes Nurse to join our team within the DGS Community, based from Livingstone Hospital in Dartford, Kent

You'll provide a link between GP and consultant led specialist teams and provide education and support both to people with diabetes and those who care for them, ensuring the delivery of a high quality and productive service based on best available evidence

Located just off the M25, there are great access points and just a short distance from Bluewater Shopping and Leisure Centre, you'll be able to take advantage of the ample coffee shops and food places nearby

This is a community based role that operates Monday to Friday

Main Responsibility

As a Diabetes Nurse you will be responsible for communicating highly sensitive information and bad news to patients and carers in an empathetic and supportive manner empowering the patient to manage their condition. You will need to be accountable for organising own workload within available hours to make sure patients receive the highest level of care.

You will work alongside the other Diabetes Nurses and Healthcare Professionals to promote the health and wellbeing of people with diabetes.

The Ideal Candidate

The post will have Nursing Degree with NMC Registration with at least 2 years post qualification experience at Band 5 or Band 6 of caring for patients with Diabetes in  in both Acute and Community clinical settings.

Evidence of post qualification CPD in Diabetes Care and Management and experience in Working with patients in groups and delivering education is essential. 

This is a community service, a full UK driving licence and access to a car is needed to fulfil this position.
