Band 6 Community Dietitian

Job Introduction

Make a meaningful impact on community health across Swindon and Wiltshire.
In this role, you’ll deliver high‑quality, effective dietetic care to adults in a variety of community settings. You’ll support patients in care homes, community clinics (through face‑to‑face, telephone and video appointments), community hospital inpatient wards, and through domiciliary visits—including some patients receiving home enteral tube feeding.

You will also play a key role in group education, supporting sessions for patient groups such as those with respiratory conditions, and providing training for healthcare staff within HCRG and local care homes. As part of our established, forward‑thinking community dietetic team, you’ll contribute to shaping responsive and dynamic nutrition services across the region.

Base locations
You will be based at either Eldene Health Centre, Swindon, or Chippenham Community Hospital, with the option to work from home as part of our flexible community service. Team office spaces in Eldene, Chippenham and Wilton are available for collaborative work, and some on‑site time is encouraged to support your learning and development alongside colleagues.

Our team provides face‑to‑face clinics and inpatient cover across community hospitals and health centres in locations including Chippenham, Wilton, Salisbury, Amesbury, Warminster, Melksham, Trowbridge, Devizes, and Savernake. Wherever possible, your main working area will be aligned with your base, while virtual services support patients across the entirety of Swindon and Wiltshire.

Key Responsibilities

  • Independently manage a specialist clinical caseload, including full assessment, nutritional diagnosis, treatment planning and ongoing monitoring.
  • Use clinical information—including biochemistry, anthropometrics and medical history—to make informed, evidence‑based decisions about nutrition care.
  • Communicate complex and sensitive information clearly and compassionately, using motivational and counselling skills to support behaviour change.
  • Ensure informed consent is obtained before any intervention and tailor dietary advice to each patient’s individual circumstances.
  • Work with catering teams to support menu planning, nutrition and hydration initiatives, and safe food provision in care homes and inpatient settings.
  • Liaise with internal and external partners—such as GPs, community teams, SLTs and support workers—to ensure coordinated and holistic patient care.
  • Refer patients to other services when non‑dietetic needs are identified or when specialist input is required.
  • Provide specialist dietetic advice to healthcare professionals, recommend nutritional supplements, and advise on medication or nutrient interactions.
  • Carry out accurate dietary analysis and ensure safe use of anthropometric equipment.
  • Provide dietetic cover across identified sites as required.

Education, Leadership & Service Development

  • Plan and deliver training programmes for healthcare staff and patient groups (e.g., pulmonary rehab, diabetes, nutrition risk screening).
  • Build strong working relationships with community teams to promote best practice in clinical nutrition.
  • Contribute to service development, policy updates, quality improvement, and evaluation activities within governance frameworks.
  • Support safe and effective workload management, including incident reporting and risk management.
  • Participate in appraisals and maintain ongoing professional development in line with HCPC and organisational standards.

Professional Standards & Record Keeping

  • Maintain accurate, timely clinical records in line with HCPC requirements and organisational systems (including SystmOne).
  • Uphold professional codes of conduct, safeguarding, confidentiality and ethical standards at all times.
  • Support managers by using resources efficiently and highlighting any budget concerns promptly.


Essential Requirements

Qualifications & Professional Standards

  • Degree in Nutrition and Dietetics (or equivalent) with eligibility for HCPC registration.
  • Evidence of ongoing CPD and commitment to maintaining professional standards.

Experience

  • Experience in NHS inpatient and outpatient dietetics.
  • Experience using MUST or similar nutrition screening tools.
  • Experience working effectively within a multidisciplinary team.
  • Understanding of the responsibilities and scope of the role.

Skills & Competencies

  • Strong oral and written communication, including presentation skills.
  • Effective problem‑solving, organisation, and time‑management abilities.
  • Able to work autonomously using initiative, as well as collaboratively within a team.
  • Confident with IT systems and electronic record keeping.
  • Flexible, adaptable, reliable and able to manage interruptions or changing priorities.
  • Attention to detail with a positive and motivated attitude.

Other Requirements

  • Car driver with a valid UK licence and access to a vehicle for work purposes.

Experience (where applicable)

  • Experience supervising or supporting students, junior staff or preceptees.
  • Participation in clinical governance, audit, or service development activities.
  • Broad clinical dietetic experience relevant to the role.

Desirable Criteria

  • Clinical supervisory skills or training.
  • Membership of the British Dietetic Association or specialist interest groups.
  • Skills in behaviour change or motivational interviewing.
  • Experience working in community dietetics.
  • Completion of the PENG course (or equivalent clinical nutrition training).
  • Demonstrated leadership skills or leadership training.
