Job Introduction
We have an exciting opportunity for a passionate and driven Band 5 Staff Nurse to join a dynamic healthcare team focused on delivering high-quality, patient-centered care within the Virtual Ward setting, based at Sheppey Community Hospital and covering the SWALE area.
This is a great opportunity to be involved in the delivery and development of a new service that supports patients to recover from or manage their illness and enable them to stay healthy and well for longer.
As part of this team, you will work closely with the Senior Sister or Team Sister to assess and plan patient care, implementing and evaluating treatment in collaboration with patients and escalating any changes in condition as needed.
Main Responsibility
As a Virtual Ward Staff Nurse you will:
- Participate in assessment and care planning of patient care in conjunction with the Senior Sister/Team Sister on the virtual wards.
- Implements and evaluates care and treatment in agreement with the patient and as defined in the care plan – escalates any deviation from the care plan or change in condition to the Senior Sister/Team Sister/Charge Nurse /GP accordingly
- Communicates effectively with patients, carers and all members of the multidisciplinary team
- Prioritises daily work in line with clinical / patient need
- Maintains accurate and legible patient records in line with professional and organisational requirements – this includes electronic patient record system
- Implements and works to ensure evidence-based care / best practice
- Actively participates and contributes to team meetings and daily handover

The Ideal Candidate
The ideal candidate will be / will have:
- A 1st Level Nurse Registration (registered with the NMC)
- Evidence of continuing professional development
- A Mentorship/teaching qualification (within first 2 years of post)
- Educated to degree level
As this is a community based role, a full UK driving licence and access to a vehicle for work purposes is essential.
