Job Introduction
We are looking for an enthusiastic Band 5 Staff Nurse or Therapist, with knowledge and experience in discharge planning and pathways, to join our Community Transfer of Care Hub!
In the Community Transfer of Care Hub you will manage all referrals into the Discharge to Assess Pathways across Dartford, Gravesend and Swanley. You will work alongside a talent multi-disciplinary team of alongside integrated discharge nurses, therapists, rehabilitation and healthcare assistants and an administrator to assist in discharge planning across acute hospital services.
Gravesham Community Hospital is located a short distance from St Georges Shopping Centre and is surrounded by well-known food establishments and local coffee shops making it an ideal location to be based from.
Whilst you will be based at Gravesham Community Hospital, you will be required to travel to local acute hospitals to provide in-reach support to meet the needs of the service and you’ll also be expected to visit patients in their homes and in care homes to undertake assessments in line with our "Home First" approach. Therefore, a driving license and access to a car for work purposes is essential for this role.

Main Responsibility
As a Band 5 Staff Nurse or Therapist you will:
- Undertake patient care needs assessment to identify on-going support required for a safe acute hospital discharge
- Visit patients in their homes to undertake face to face assessments of patient needs.
- Source appropriate care provision in the correct setting to meet these needs.
- Ensure sourced care provision is Implemented from the point of hospital discharge.
- Communicate effectively with patients, carers and all partner organisations
- Maintain accurate patient records in line with professional and organisational requirements
- Implement and work to ensure evidence-based care/best practice.
- Initiate health promotion activities with individuals, patients, and carers.
- Provide in-reach assistance to the acute hospitals by providing expertise in community services to improve discharge and flow for pathways 1, 2 and 3.
- Undertake mentorship training, facilitate the teaching and learning experiences for all students and colleagues.
- Be responsible for liaising with external agencies to expedite Transfer of Care Packages and Nursing Home/ residential Home placements.

The Ideal Candidate
Our ideal candidate will have / be:
- Registered Nurse (RN) or registered Therapists (OT / Physio) with current live PIN on NMC register or HCPC register
- Be able to make decisions under pressure
- Able to act autonomously
- Ability to manage conflict
- Ability to communicate at every level and effectively and sensitively in difficult situations
- Able to accurately assess, monitor and document risk
As this role requires travel , a driving license and access to a car is required for this role.
