Are you a dedicated and compassionate Registered Nurse looking to make a meaningful difference in people’s lives within their own homes? HCRG Care Group is looking for an enthusiastic Band 5 Community Nurse to join our dynamic District Nursing team.
About the Role
As a Band 5 Community Nurse, you will be an essential part of our 24‑hour district nursing service, delivering high-quality, holistic care to patients in their own homes. Working under the direction of the District Nursing Team Manager/Leader, you will:
- Carry delegated responsibility for the assessment, planning, implementation and evaluation of evidence‑based nursing care.
- Provide skilled clinical care that supports patients to maintain independence and quality of life at home.
- Supervise and support team members in the absence of the District Nursing Team Leader when required.
- Work collaboratively with multidisciplinary colleagues to ensure safe, coordinated and person‑centred care.
About You
We are looking for someone who is:
- An experienced Registered Nurse with a passion for community healthcare.
- Confident in delivering holistic, evidence‑based nursing care.
- Able to work autonomously while remaining a strong team player.
- Skilled in communication, assessment and clinical decision‑making.
- Flexible, compassionate and committed to delivering high standards of care.
