We’re looking for a motivated and caring Band 4 Community Neuro Advanced Rehabilitation Assistant to join our new community team in Swale. This is an exciting opportunity to be part of a dynamic multi-disciplinary service delivering person-centred, high-quality rehabilitation in the community.
You’ll work under the guidance of registered therapists to provide hands-on support and rehabilitation interventions for patients in their own homes. Your role will also include assisting with basic assessments, maintaining service equipment, and being a key point of contact for patients and families.
Main Responsibility
You will support patients by delivering individualised, goal-directed rehabilitation programmes prescribed by physiotherapists and occupational therapists. This includes helping with functional activities, mobility exercises, and basic assessments following Trusted Assessor training. You’ll monitor patient progress, keep accurate records, and liaise with other members of the multidisciplinary team to ensure coordinated care. The role also includes prescribing and maintaining rehabilitation equipment, completing administrative tasks, and travelling to patient homes as part of your community-based duties.

The Ideal Candidate
We’re looking for someone with:
- Experience in NHS community rehab or as a Band 3 assistant
- Knowledge and experience of neurological rehabilitation
- NVQ Level 3 or equivalent in Health & Social Care
- Strong communication and interpersonal skills
- Ability to work independently and manage their own caseload
- Commitment to safeguarding, information governance, and safe practice
- Basic IT skills and confidence using digital systems
- A full UK driving licence and access to a vehicle for work
Desirable:
- Trusted Assessor training or equivalent qualifications
- Group facilitation experience with Allied Health Professionals
